Granted I have good gear and these guys weren't equipped with primo stuff but they didn't even get through my armor - really all it takes is patience and paying attention when you're in combat since wildly flailing about can actually allow enemies to beat you down. In other areas it makes things easier/more convenient but as long as you have the ability to master strike from Bernard the game really isn't too difficult (about half an hour ago I killed 5 enemies who ambushed me on the road at night while my hardcore Henry was drunk (past the halfway point, blurred and swirling vision).

Like I mentioned in my other comment the stealth skills and speech (I guess drinking too for the Johanka DLC bs) are the only areas where skill level really matters in my opinion.
